Hey, everyone!
Could yoy please explane what docs and applications my relatives in Ottawa must take with them to CIC office to apply for my caips notes?

Your relative does not have to go to the CIC office. Please follow the following procedure to get your CAIPS notes.

1) Get the Access to Information Request Form (TBC/CTC 350-57) at this address :
http://www.tbs-sct.gc.ca/tbsf-fsct/350-57_e.asp

Fill the Access to Information Request Form and ask your relative to sign it.

2. Write a consent letter to authorize the CIC to send your CAIPS notes to your relative. Mention your file number, date of birth and the visa office where your file is being processed on this letter.

3) Your relative should provide a copy of his/her PR card or passport to prove legal status in Canada.

4. Add to these documents a money order for 5$ CDN payable to Receiver General of Canada.

5. Send a) information request form; b) consent letter; c) proof of legal status; and d) 5$ to
