Hello~ guys~~
I am trying to apply for my wife´s PR visa.
In the IMM5491, there is the sentence as below.
An original "Option C Printout"of your and your co-signer´s last Notice of Assesment for the most recent taxation year.
To obtain this printout free-of -charge from the Canada Revenue Agency, call 1 800 959-82813
If you do not provide this printout, provide an explanation on a separate sheet of paper.
In my case, I stayed in Canada just one month and have stayed other country now, after I got my PR visa.
What´s the"Option C Printout"?
In my case, I need it??
One more question...
If I see the IMM5491, I have to mark the right small boxes with "X", but, some documents have nothing to do with me.
Then, I can leave that boxes blank? I wonder ... coz there is not enough space to write"N/A".

I found this on a different forum: "It is a tax assessment from all walks of government and financial life. Option-C is like a transcript, that would document your income tax, EI, CPP/QPP, CCTB, student loans, Old Age Pension, social assistance if any taken and the list continues. It reflects an individualīs marital status as of that tax year, name, DOB, SIN and address with Canada Revenue Agency. CIC asks for such ones because it is more efficient and effective way to figure out financial standing pursuant to federal regulation of an individual."
