FBI Fingerprinting Card Questions

Subject: FBI Fingerprinting Card Questions
  Hi everybody

Got a letter from Buffalo consulate to send FBI and RPRF.
Went to UPS store to get my fingerprinting done.On the card there are few things like:
ORI
Your No.- OCA
FBI No - FBI
MISCELLANEOUS No- MNU

The guy who did my fingerprinting asked me to leave these blank.Is it ok?

And my wife has no social security # as she´s on H4 (dependent visa).So do i put her passport number in the ´Miscellaneous No´ space.?

Any help on this greatly appreciated...

I would say ok to leave these spaces blank. The instructions from the FBI website only mention having to provide your name, date of birth, and place of birth (and of course your fingerprints).

Here are the instructions (http://www.fbi.gov/hq/cjisd/fprequest.htm):

Obtain proof of identity, which consists of a set of your fingerprints (original card, no copies), with your name, date of birth and place of birth. Fingerprints should be placed on a standard fingerprint form (FD-258) commonly used for applicant or law enforcement purposes.

* Include rolled impressions of all ten fingerprints and impressions of all ten fingerprints taken simultaneously (these are sometimes referred to as plain or flat impressions.)

* If possible have your fingerprints taken by a fingerprinting technician (this service may be available at a Law Enforcement Agency.)

* Previously processed fingerprint cards will not be accepted.

My spouse is on H4 visa too, we put the ITIN number as there is no social number. It worked for us.
