Most graduate schools require an Honours degree, which is typically a 4-year program. There may be exceptions. Some schools will take a few years of work experience in your field of study as an acceptable substitute, for example. But fresh out of school, I´m not quite sure if you can find a place to go for grad. I dunno, maybe get perfect on the GRE? :D

It depends on the program and the school.

As long as you have your degree and good marks, you should be able to go to grad school. The "honours" requirement is only for a few subjects like mathematics or economics.
