Hello guys,
Finally, I have completed my application to sponsor my wife and ready to send to CPC-M. I live in Canada and wife lives in India. Today I paid the required fee to sponsor my wife, over the internet and got printout of Copy-2 which needs to send along with application.
Now I have a question regarding filling up the Copy-2 of Receipt.
1. In Name and address column. Do I need to give my wife(sponsored person) name and address which is outside of Canada? Or name and address of myself(sponsor)?
2. In Client date of birth column. Whose information is required, myself (sponsor) or wife (sponsored person)?

You are the payer, therefore all information requested on the form pertains to you.
I am sponsoring my spouse and included my information, the sponsor, on the receipt.
