Hi
I got my passports (me, my wife & 2 yr old son) stamped in March, 2005 and now I am planning to land in Canada from Buffalo by car.
During stamping along with COPR, I had received "Immunization Record - applicants under 16 years of age" document. It says applicant´s responsibility to have this form completed and certified by a qualified health professional before departing for Canada.
With whom I should get certified this document. With regular my son´s Pediatrician doctor or Canada site listed doctor?
Can any one please clarify me, if they already done it.

(in reply to: Immunization record) i just went to the "urgen care" clinic and ask them to transfer my son´s shots record from blue school form on the sheet from Buffalo office. They just simply copy it and sigh it.
that is all what i did. i think you can go any clinic and ask them to copy your son shots and sigh it.
