| Subject: Consent for CAIPS |

When requesting for CAIPS notes through a Canadian resident you are supposed to give consent to that person.
Can someone tell me how the consent should be given? Is it through a letter addressed to CIC? what is the correct way to do it? Please give me as much detail as possible

you need to have IMM 6475 form. you can acquire it ftom CAIPS.com KK
