Hi,
Please can you tell me, which Form No. is required if we want to make an amendment to the name.We added the family name to our daughters first name. The form that we downloaded says that if the error was made by the immigration office only then you can use this form. I cant find the form that we have to submit.

Not sure where you were looking - but this form can be very easily found on the CIC web site.

Yes I have already downloaded this form... but it says that this form will only be used if the error was made by the immigration office...
Our document is not an error made by the immigration office but it was there before we entered canada.
You see, the amendment we want is to our daughters name, before landing here there was no family name on my daughters passport just her first name. Now we got a new passport for her adding the family name to her first name. For getting it changed on the landing papers the form that I downloaded and the one that you are telling me is not the same.
Because the form says that this amendment will only be made through this form if the error was made by the immigration office...
I hope I conveyed my message properly and hope that someone can help me.
