Its been almost a year that I´m waiting for response from CIC visa office. Now my wife is pregnant. Do I need to inform CIC visa office?

No, you don´t have to inform the CIC visa office until the baby is actually born.

But we already finished our medical examination. In this case we are waiting for decision to be made. So What I have to do?

If you receive your PR visas before the baby is born and can land in Canada and officially become PRs before the baby is born - then there is nothing you have to do.

If the baby is born before you receive your PR visas or before you can land in Canada, then you will have to notify the VO about the baby´s birth and add your baby to the application.
