Subject: Paperwork that I need to take to medicals
  There is a list of things that I need to bring with me when I go to get my medicals done. One of the things that they asked for was the immigration paperwork that they´d need to fill out. I called the 800 help line (CIC), and they told me that they´d send me a link to my email that would tell me what I need as far as that paperwork goes. The description of the link that they sent me, read as follows : Applying for permanent residence from within Canada: Spouse or common-law partner in Canada class - Medical Requirements. The problem is, when I click the link, unfortunately, all I get is this message: HTTP Error 404 – Not Found. So I´ve tried to research online what forms I might need, and I feel like I have been sent running around in circles, and now I´m not sure what I need! Appendix C looked like the right thing, but then so did IMM1017 SCL.

I am hoping that if I explain my situation, and how I am filing, someone may be able to lead me to the right place of where to find what forms I need to take with me when I get my medicals done. My biggest fear is bringing the wrong thing, and then having the process stalled.

Okay, so I have been in Canada for over 4 1/2 years with my fiance. He will be sponsoring me as his common law partner. He is a Canadian citizen, I am an American citizen. We were originally going to file inland, but were told that outland would be a faster way to go (to my understanding, similar paperwork that just needs to be sent to a different part of Canada to be processed). We are applying for permanent residency. Please feel free to ask if I have left anything out that might help to determine what paperwork I will need to give to the DMP to fill out on the day of my appointment.
