where can i get a format of the english proficiency declaration? do you know any website/links whre i can get one. thanks.

Do you mean your letter stating that you are proficient in English? There is no standard format; just write your experiences and education. My understanding is that it is very difficult to prove/convince if you are not a native English or French speaker (even this, apparently, is no guarantee you will not have to take the IELTs).
