Subject: UK poliice certificate
  I requested information about Prosecution/Conviction History from UK. They send me 3019 form which is about asking for Police certificate. When I read Canada Visa office specific instruction for permanent residency to Canada, they are not requesting Police Certificate. They are asking for 3019A form and Prosecution/conviction history. I am totally confuse. The officer from UK office told me that, this is the letter they are providing. After all I couldn´t find any solution to get my prosecution. Conviction history from UK. Please let me know if anyone has any idea about it.

Ayse,
I believe what you needed WAS a "Subject Access" form which was available via certain police stations (Area Main Stations for where you live - the info was on the web)- this form stated whether the UK Police held any information on you. (we got ours in on the last day they could do it! (Biggleswade, Bedfordshire))
Apparently this has just been changed - so if that is the NEW form then so-be-it (All this changed from the 1st of April 2008 I think - and is now done by another agency??)Otherwise contact the police office that used to do it and ask them.
