| Subject: missing medical form |

hi ,
we recieved IA along with medical request . the only problem is that they forgot (?) to include a form for our 16 month old. someone on this forum told me to go ahead with doctors appointment but when I called the doctor he said he wasn t sure it would work out ( he never heard a similar case )
We send a fax to CIC about this and are waiting for their answer .If anyone went through the same thing, we would really appreciate if they agreed to share their experience with us.

Are you sure that they forgot to send? Did you inform them of your baby? If it is their fault, then they will send a new form for the baby. but if you failed to inform them of your new born, I think you will be in trouble...
A better way of communication is to e-mail your immigration office

yes we did inform them (in reply to: missing medical form)
we have provided everything from birth certificate to paaseport for both of them .During the call / interview on may 30 the immigration officer asked about both our kids :one was born in the us and the other in france .
We faxed and e mailed them on monday to let them know about this missing form.

I have seen people submitting medicals before they were asked to do so I do not see any reason why you cannot go to the DMP and ask him to include your baby in the medicals. Another way is write to CIC and request them to issue medicals for your baby also.
