Subject: Declaration in Lieu of Guarantor
  Hello, I am a permanent resident in Canada. I will be traveling to Europe in the end of April and I am currently in the process of redoing my passport application since it will expire in June.

I do not have any personal relationships with a professional as my guarantor therefore I must fill a PPTC 132 form.

Has anyone had to do this and can you please tell me the process and steps to this procedure? I will be going to the passport office in Scarborough, Ontario on Monday and I would like this procedure to go smoothly.
