I would like to find out what are the photo requirements for a PR card?

1) Photo dimensions: 35mm (1 3/8") wide x 45mm (1 3/4") long
2) Head - chin to top length: between 24mm (1") and 35 mm (1 3/8")
3) Color or black and white
4) Taken with 6 months
5) Bear date photo was taken on back of one (from machine)
