Subject: Do I have to wait for medical instruction?
  It´s been 3.5 years since I submit my application. Last year, I received the letter posted on 24 November 2005 from CIC Singapore stated that I have 69 points.
They also mentioned that they will issue the medical instructions sometime in 2006. However, it´s been 5 months now, I haven´t heard anything else from them ever since.
Do I have to do medial test right away? or do I still have to wait for their medical instruction first?
Should I write the letter to CIC in Singapore and ask for formal medical instruction letter from them?
Should I pay landing fees in order for faster processing?

Yes you have to wait for the instruction. The reason the Medical Practioner only have 4 Medical forms, while you need 5 . And the first form was the one they are going to send you.

You can write to CIC singapore. I found that paying the landing fee right the way can save you 1 month processing time, but that was only my subjective opinion.
