Canada Address on Medical Form: Applying from USA

Subject: Canada Address on Medical Form: Applying from USA
Hi,

I will appreciate if someone could let me know that do we need to fill Address in Canada on Medical forms. Is this mandatory? I live in USA and applied at Buffalo.

I don´t think so. I mean you are not in Canada, why should you have an address?

as far as I remember, there was a space for "intended place to live" on the form, and the form came that field filled (ON, BC, ....)

Hi Rookie,

Thanks for the quick reply. Apart from what you mentioned there is another column where they ask for Contact Address / Person in Canada. This is just below our current US mailing address. Please let me know if you can look at the photocopies of your forms, in case you have.
