Here are some of the ways how to get them:
1. Some local police stations have it, call ahead to know.
2. FBI definitely has it, call their customer service # to know how to ask: 304-625-5590
3. Private investigation firm (this is what I used)

where I live, any UPS store can provide you with the forms and fingerprinting.
