| Subject: which number I can fax for error in medical form? |
Can anyone tell me which number I can fax to if I find error in the medical form. I lost my medical exam instruction.

You can correct yourself small errors like wrong address while CIC has correct one or spelling mistake while CIC has the correct one or change of name while CIC knows about it already. I did all these things and it worked. Do the correction without hiding the mistakes on the form ie cut it off with one line and write the correct ones.
